Landmark Health’s In-home Model Extends Life and Reduces Cost
HUNTINGTON BEACH, Calif.--(BUSINESS WIRE)--Dec 3, 2019--
Landmark Health and its affiliated medical groups ( Landmark ), a pioneer and current industry leader of value-based, in-home comprehensive care for complex, chronic patients, recently shared key results of their clinical model as a result of extending care into the homes of thousands of patients over a multi-year period.
Landmark is uniquely positioned to share real-life outcomes versus the model predictions of newer entrants into this field. The results are based on a retrospective cohort study of 36,393 Landmark Medicare Advantage patients who were attributed to Landmark between January 1, 2016 and June 30, 2018.
Patients engaged with Landmark had a 26% lower risk of death 1 compared to non-engaged eligible members over an identified sample period.
Even for patients who did not elect hospice, Landmark’s interventions (including a focus on palliative care at the end of life) are associated with a 20% reduction in costs 1 in the last 12 months of life.
“We’re not just doing work that improves the lives of patients,” states Dr. Michael Le, Landmark’s co-founder and Chief Medical Officer, “we’re doing life-saving work.” Landmark’s research shows that for patients who receive Landmark’s care, mortality rates are significantly lower than those who do not receive Landmark’s care.
Landmark’s Model of Care
Landmark’s high-touch, interdisciplinary model bridges the gaps in the current fragmented healthcare landscape, providing 24/7 care access to prevent and manage exacerbations and complications of chronic illness, as well as limit burdensome symptoms.
Landmark’s model is focused on consistent communication across the longitudinal care system, including PCPs, specialists, inpatient teams, home health, and hospice agencies. This focus reduces high-cost sentinel event spending, mitigates downstream costs, and improves patient satisfaction.
Dr. Le concludes, “It’s really gratifying to know that we are helping patients to live longer in their homes surrounded by their families with higher quality of life.”
Landmark Health and its affiliated medical groups (Landmark) partner with health plans and delivery systems to bring patient-centric, in-home care to complex and chronically ill populations. The company bears risk for more than 100,000 lives across 14 states. Landmark provides access to care 24/7 to patients and their families. Its value-based model relies on fully-employed, local multidisciplinary care teams to help drive long-term outcomes for patients by bringing medical, behavioral, social and palliative care to individuals, where they reside and when they need it.
